Led by a team of experts in diabetes education, including nurses, dietitians and certified diabetes educator professionals, the Diabetes Self-Management Education program provides fundamental knowledge for diabetes care, including:
- Disease process.
- Risk factors.
- Signs and symptoms.
- Treatment options.
- Nutrition overview.
- Label reading.
- Benefits of exercise.
- Preventing complications.
Following a 90-minute overview course, participants will receive ongoing support and counseling through one-on-one appointments. Topics covered usually include:
- Goal-setting.
- Meal planning.
- Medical nutrition therapy.
- Meter training.
- Testing schedule.
- Effects of food on glucose.
- Hypoglycemia.
- Injection teaching.
- Emotional aspects.
- Foot care.
- Medication options.
This program is recommended for:
- Those who are newly diagnosed with diabetes.
- Those who may have diabetes for many years but have never attended an education class before.
- Those who have been to an education class but need a refresher.
